When a near-accident took place during maintenance in one of the plants, we were asked to produce a video that reconstructed the incident, as a teaching aid for new employees. The video shows the events that lead up to the incident, with the real “actors” in the control room and in the installations. It clearly shows how the slightest oversight can turn routine maintenance into a dangerous situation.
